             Documenting a patient's appointments, services, tests, and treatments is referred to as medical coding. For the purpose of creating insurance claims and patient bills, medical coders extract billing information from clinical records. In order to choose the correct billing codes for a patient's particular diagnosis and treatments, they must first examine the clinical paperwork. In hospitals or doctor's offices, medical coders must work.
